In Re; A General Question

To whom it may concern:

I was directed to contact your department, regarding a troubling issue that -- multiple times, countless even! -- I have attempted to retrieve funds provided to me by UHG, which, unfortunately, had expired, whereupon I was informed that the funds were forfeited, despite a complete lack of documentation provided to me, at any point in time, regarding this matter, not to mention the ostensible illegality of such a move by an FDIC member, such as the company providing such cards.

Additionally, I was told that the cards, physically in my possession, could not have been issued by UHG, despite the fact that Optum is clearly named on the cards, and Juan, a supervisor from UHG's Escalations Department, informed me that no other company was affiliated with Optum, save UHG. Moreover, I was told that I had only been a member for approximately one month, during which time, my Medicare Drug Plan would not have issued any cards, and, if it had, they would have been from Visa, not MasterCard, whose logo is clearly seen on all of the cards.

Given these details, I sincerely hope that a favorable resolution will be reached, preferably in an expedited manner, so that I may put this entirely distressing experience behind me.
