Verizon Wireless - To demobilize my stolen phone

My iPhone 6s was stolen May 20th from a public place. I filed a police report. Got another iPhone (13). I have been getting hacked in so many apps still. Even after sims card disconnected. I have only used iPhones and been with Verizon for 11 years. You all have me listed as having a Motorola phone bec as use that is what my sims card was transferred into. Please help me locate this iPhone and disconnect it. Please and thank you.
