Doctor Holmes denied me exercise the ‘right to try’

I got sick with Covid at Thursday, December 23. On Sunday I went to ER of the Lutheran General Hospital in Des Plaines, IL, which happened to be a complete waist of time and efforts.

I was discharged and tried to reach my Primary Care doctor starting this morning, asking to provide prescription for COVID therapeutic medication. Finally doctor Holmes called me and told that i have no rights to requests the therapy treatments.

User's recommendation: I’m looking now to switch my PCP and wood not recommend Lutheran General as the health provider.

Bought their software and disc. Never received the disk and complained and received the $40 back for the disk and then they turned my software off so I could not use.

Cannot contact them!!! Have emailed them at least 10 times and complained to PayPal and still no communication with them {{Redacted}}

Software does not work

I have had the software for a few years, there was no upgrade path, so when the 2012 version was released I repurchased the Medical form software. Recently my insurance company told me that I needed an updated version of the 2012 Form so I purchased the software, they sent me older version of software.

I alerted them, they sent me a new link, that brought me to software that I already owned. Not the version my insurance company requested. I alerted them they sent a newer version. This was still the wrong version and does not work with the insurance companies I bill.

I have been trying to contact via e mail for either the correct version or a refund but they have not responded. They have no phone number(should have been a clue to the quality of service)

Medical form software cms-1500 doesn't work

I purchased a new version of the cms-1500 software from medical form software that was compatible with windows 7. I purchased it in April, 2012.

I have used the version for Windows XP. It worked fine for two years. After I ordered the new version and downloaded the software as well as bought the back up disk for windows 7, I was having trouble printing out the form but on occassion it would work. Last week it would not print out the form.

I e-mailed the "company" when I starting having trouble with the printing and after it wouldn't print. I have e-mailed 3 times and have called but have not received a response. I don't know what recourse I have at this point.

Don't buy the download or the backup disk. It is a rip off.

I don't know how this guy stays in business. I finally gave up, lost my money, and found another program that is very good.

I too am having a major conflict with Medical Form Software. The forms will not print.

I emailed my concern and was told to watch the vidoes for adjustments. After viewing said videos, I still could not print the forms; as a result, I requested a refund. The response was "no refund. You agreed to this when you purchased the software".It was very negative in tone.

Medical Form Software will never get another penny from me! :(

Cms 1500 form

I bought 1500 form for $69 from medical form software on line. I hardly used it and then it said: "Gamma Quest has quit working." There is no ph# for this company.

I emailed them with questions and it would always take 3-4 days before any reply and it told me to uninstall it and that they would send me another link. I installed it and two weeks later, the reply was: Buy a new one!

I really felt that this was a one man hoax of a business and not a reliable company. I would never buy anything more from them and really their customer service did not exist.

this software has serious Hippa breach issues, save your self by not using this, some of the patient data is uploaded on uploading sites.

I wish I had read this before. I used the same company three years ago with no problems, however this time I am being told that my demo period has expired when I ordered the whole system again due to a corrupted data base.

:( I too purchased this software and received the same error message. I emailed their sales email, since they have no phone number listed on their site.

The email was returned saying the recipient inbox was full. Looks like this is a scam.

AVOID MEDICALFORMSOFTWARE.COM!!!! I'll post some more stuff on FB, Twitter, and Yelp about this scam.

Do not download or order CMS-1500 software from MedicalFormSoftware.com. The customer service is non-existent. I e-mailed their so called support 3 times with the same question over the course of 3 months and to this day have not gotten a response.

This software has a database which holds all of your patient's billing info. I had been asking how to back up this information in order to avoide a disaster if my computer ever crashed. Twice I asked trying to be proactive. 7 hours ago, I asked a third time because my computer IS CRASHING. I explained I urgently needed to know how to keep my database safe / stored to usb. Still.... no response.

So...either it's possible and they just don't care to check their emails or respond. Or it's impossible to back up this information and they didn't want to tell me. Obviously, when working with medical billing - the option to backup hundreds of patients data is crucial.

Their website states .."All of your Claim forms will be stored in the "Easy to use" Database. With our Software, you will never have to type in any information in a second time, ever !"

So I guess what they meant to say is...as long as you never have computer issues. Shame on me for giving these people $100 for a download of their software and a hard installation disc. Money well wasted.

This software violates HIPAA rules altogether and looks like it is uploading information to their server too that HIPAA Breach.

Avoid this to save yourself from hipaa breach penalties.

i've been duped....i had my company spend money on this junk....as mentioned already, it looks like these guys are out to steal your money. I downloaded the software and when I went back to open the application a week later got an error message that the "master data-base is corrupte/application will close".

I made an inquiry with them about upgrades to the Windows 7 version and did got a very curt reply from "dave." I would have bought the upgrade if he had been even a little polite, but his response inspired me to look elsewhere and I discovered that I could do without their software all together.

By the way, if you are locked into using this software for some reason, and need to back it up, look for the database file in C:\Users\YourUserName\AppData\Roaming

I'm assuming these jokers just start up under a different website/name when they make enough people unhappy under their current one. It's a shame they get away with being so belligerent to customers.

Just had similar experience re backups, ultimately did file complaint with BBB today after final email message from company saying, "U are a failure.. ***

This company is definitely one to be avoided.

I am looking into whether they have a business license anywhere. If so, that should be reviewed in light of responses ranging from silence to terse unhelpfulness to outright rudeness.

Update: Contacted their "sales" email address and posed as an interested prospective buyer of their software. I received a response within minutes.

In the process, I asked the back up question and was told that the data cannot be backed up. When I revealed that I was a current customer they had been ignoring for 3 months I was responded to with "How bout u *** off". When I replied the response would come in handy as I filed complaints about them with the BBB, etc, he/she responded once more threatening me that they have control over my software/computer, etc.

They are not BBB accredited and already have a grade of "F" on their website. Wish I had checked there before purchasing.

MedicalFormSoftware.com called me an *** because they couldn't fix their software glitch.

When I tried to get assistance from MedicalFormSoftware.com on their CMS-1500 software, they became progressively more belligerent and eventually called me an *** for wanting the name of a real person to contact. Evidently if you download IE9 it will mess up the Excel spreadsheet template designed so that the data you enter will not align correctly with the (expensive) CMS-1500 forms. Their only answer was "guess you'll have to buy a new copy" and "send us your computer to see what you did to our software." After repeatedly asking for a contact they called me an *** They never did give me a contact.
