My comdata card expired...need new one...i never received the updated card in the mail...now having problems tying get a reference number .. I never knew I had.that is why I am trying to get help to order a new card...i have called several phone numbers...there should not be so many numbers to call and get no help.

Do I have a reservation or not? My doctor's appointment is tomorrow.
I went to modiva website where I made the reservation, now it says my ride wasn't approved. So I cancel my appointment since I won't have ride according to the website. Now I received an email reminding me of reservation tomorrow. After they said I wasn't approved!
So now I have a ride and no doctor appointment because you can't make up your mind! I'm a cancer patient and need to see my doctor.
Mind you, my doctor is an hour and a half away! Thanks!

User's recommendation: Don't Use These People! I've cancelled 2 cancer treatment appointments because they said my reservation wasn't approved. After I cancel, they email with a reminder of my ride reservation! Really! Get your head out of your butts and pay attention!

Preferred solution: Acknowledgment but I did send you proof from January and acknowledge and I wish I can get my reimbursement from January February and March

I need help activating my card it says I need a number to active it but there isnt one on this letter
Still without transportation going on 3 weeks
- - It took weeks to arrange a taxi for medical visits.
- - The 2015 form's fax number was wrong and it was faxed with no review while Medicaid covers transport.
Been trying for 3 weeks to get the taxi to pick us up we been to the Drs and had the 2015 form faxed 6x .we call every day and keep being told our form has to be reviewed..we cannot afford public transportation and need to be at the clinic 6 days a week as well as other Drs appointments.
We have called medicaid been told we are covered for transportation however modivcare has still not reviewed our 2015 form ..
By the way the fax number printed on the bottom of the 2015 form is the wrong #.
Our Drs.office was faxing the firm for 2 days before someone finally told us the correct #.

The driver yelled at me saying that he called me I did not receive any calls from him he kept yelling at saying that hes going to pick me up I told him if he does come I will call the police on him he was very nasty my picke up time was 2:55 and my apartment is at 3:45 please call me at 973-462-****

I can not get anyone yo give me the reference number to get anything done with my gas mileage reimbursement card. I've not been able to access funds since february and every time i get a deposit my money is gone before I can get to an atm to withdraw it. I cant get anything done without the reference number and ive sat on hold every weekday for 7 days now for over 2 hours each time and still cant get anyone to help me figure this out and i desperately need the money.
