Illegal and fraudulent Medical practice

I went to this facility to obtain outpatient rehabilitation services for substance abuse issues. I thought this would have been the best option possible for me considering my former best-friend Caitlin Renee Golder worked here as the Medical Assistant Supervisor for all four (4) of their office locations, including her since proven false statement that they were opening approximately 12 new offices between 2022 to the present.

This included an office in Rochester, Minnesota (guess what! It doesnt exist), as well as a location in Honolulu, Hawaii (no surprise here, it doesnt exist either but their is a registered physician by the name of Dr. Kai Shin licensed in the State of HawaiiMs. Golder spent her childhood summers here with her aunt Heather so it makes sense where she came up with the name.) on the day I started treatment at this facility, I was admitted and determined to be in a mental state known as psychosis, wherein based on law passed by the Minnesota legislature, I would not have been in a fit or sound state of mind to execute any type of medical documentation.

During this first visit, Physician Nicole Solberg and Medical Assistant Supervisor Caitlin Golder had me execute a Medical Authorizatuonwhich I didnt fill out or otherwise insert information, though Ms. Golder didwhich stated she could obtain and access any and all medical records for me. Considering I didnt know what I was signing at the time of admission, I was shocked to go back and review this at a later date. Furthermore, there would have been no reason Ms.

Golder should have needed unlimited and unbound access to my medical history in order to receive treatment on an outpatient basis. After I learned about this authorization, I requested multiple times that the authorization be revoked, which went unanswered and uncured each and everytime. However, I did get falsely diagnosed with Borderline Personality Disorder, was incorrectly told what BPD actually is, and was being written prescriptions for medications that were over the counter and didnt need a physicians signature to obtain. This included a none approved FDA drug treatment option for helping with addiction cravings and for managing ADHD and it is known as NAC.

I was not told if this drug was in review by the FDA, I was not told anything pertaining to potential side effects, nor was I told if it was clinically proven safe for use by humans. I stopped going to treatment for a long while, but eventually resumed treatment approximately a year after my initial intake fiasco. It was during this next visit that I learned Kai Shin Clinics is an Allina Health organization because, after an emergency room visit in 2023 for a severe upper respiratory infection, they had my insurance data and information in their data base. Ms.

Golder works at Allina Health and did the entire time, but would take days off work in order to ensure she was at Kai Shin Clinic during my appointments. Furthermore, an acquaintance of Ms. Golder who is the reason I was a drug addict, stole my healthcare information and used it at an Allina Clinic in 2022 prior to my first visit at Kain Shin. During this incident, he setup a healthcare directive for me, which I only learned about almost a year in a half later during the ER visit for my upper respiratory infection.

This organization is probably fictitious and, if it is real, is only a labs testing facility, which Ms. Golder used to commit theft, money laundering, assist with identity theft, and gained illegal and unauthorized access to my medical history with the help of a licensed physician, Nicole Solberg, who practices family medicine at Allina Health.

Preferred solution: Fully refund for all claimed services, lost wages, emotional distress and damages, and to have Nicole Solberg’s Medical License revoked indefinitely.
