Can't get document to print

My firm filled out shows on the screen and I am able to send filled out but when I attempt to print it, it prints the blank template and not the information filled out. Why???

For doc hub pro uspos to take $10 instead you took a $120. I need my money back call me at 302922**** Or refund my $110 now please?
