Why am I getting a medication called PEPTIVA the last two months from your company and I have never asked for it. I have never heard of this medication ever in my life.
I have never heard of your company either. How did Adaptive Health get my home address and my credit card number. My name is Dennis E.
Leonard and my address is {{redacted}} and my cell phone number is 1 ( 408 ) 707-****. On top of that on 3-31-2024 your company charged me $5.99 and then one month later on 4-18-2024 you also charged me $70.63.
