Please fix this mistake... somebody plz talk to me... I need answers..?.
The Separation date and Status information provided by Swift is incorrect/. I left Swift because Swift provided me with a truck that gave me carbon monoxide poisoning:. I had tried unsuccessfully on at least 5 different occasions to have this condition repaired and Swift mechanics continued to say they fixed the problem!. The last time, on December 23, 2012, I turned in the truck and I was taken to Good Samaritan Hospital in Lebanon, Pennsylvania, where I was diagnosed with and treated for carbon monoxide poisoning (see attached)|. At that time Swift mechanic told me that he would send my truck to Kenworth for repairs on 12/26/12". In addition, after I got out of the hospital Swift Onroad told me they were granting permission for my truck to be serviced by Kenworth!. I was taken off the unsafe truck (#100329) and assigned to truck #306266?. I understood I should remove any valuables as Swift is not responsible for lost or stolen items while the truck was in shop. I also understood I am not responsible for a truck not assigned to me anymore. I packed my valuables as well as items needed to perform my job in my new truck #306266. Dispatched, I left the terminal.
When I called Jonestown terminal on December 28, 2012, Swift mechanic told me that they did NOT send the truck to Kenworth as promised and I would have to use the truck as is, repaired by Swift's mechanics (the same group of mechanics that failed to repair the truck the first 5 times it was in for repair of the same condition). I returned to that terminal 12/29/12 to return the truck I was using and brought with me a load being "T" Called there and resigned 12/30/12.
Specifically, I returned to the Jonestown, Pennsylvania terminal where the truck was located and filled out a Swift Document "Resignation" on 12/30/12. At the drivers window I was invited by "Rick" to join him in his workspace behind secured door and use his desk space. I was instructed by Rick to place the completed document in a basket to be received by Swift. A company driver sitting in the drivers' lounge was asked to use the Swift Company Van to transport me to Harrisburg, PA Greyhound Station where I purchased a one way ticket home to Ohio. The Resignation stated I was separating myself from Swift to seek other job opportunities. At the bottom of the statement I included a brief statement about the exhaust problem that my previously assigned truck #100329 had and after repeated attempts to have truck exhaust issues fixed by 3 different terminals in three different states over a span of 6 days I felt unsafe after the diagnosis of Carbon Monoxide Poisoning on 12/23/12.
I called Swift in Phoenix, Arizona after I was denied an opportunity to start immediately for a new company due to something Swift marked “yes” to on paperwork returned to potential new employer. I was told by Swift to use the website DriverFacts for all inquiries. I paid money, faxed necessary documents to individuals in order to get answers. I received an email document showing a separation date of 1/4/13. I resigned 12/30/12 and have a dated bus ticket showing I boarded a bus home to Ohio that day. Email also shows a separation status of involuntary. I worked 12/29/12 and resigned 12/30/12, That would be a voluntary separation. Email shows a DOT Drug/Alcohol violation. Again, I worked 12/29/12 and resigned 12/30/12 in good standings with Swift. I have never been approached by DOT and have no violations. I follow website procedures and apply for these items to be corrected and I was denied by Swift for any changes to take place. In the “notes” space provided, no explanation, blank, empty... I don’t know what to do.

I am sorry for being rude to the mechanics at Jonestown terminal. I am sure I offended most all of them when I so rudely addressed the situation. I spoke to onroad 17 times in a 15 day span. I was always grateful they were there to help as much as they could. I have a $10,000 truck driving education to pay for, a 5 month career with Swift, a new useless CDL License, bills to pay, 3 wonderful kids to support and no job doing what I Love to do. Onroad took calls from me 17 times in 15 days. The calls are on my phone bill. The exhaust problem... 5 visits / 3 terminals all telling me the truck was fixed. 12/23/12? I was so sick from the exhaust I went to the ER in Lebanon PA and was diagnosed with CO poisoning. ( After the second attempt to have the truck fixed I pulled over in Zainsville, Oh and was towed to Columbus). I couldn't drive it anymore. That isn't even the end of it...
That truck was taken out of my name on 12/24/12 and unless it was professionally fixed by a Kenworth Dealer I wasn't getting back into? it. (Onroad told the shop it was ok, send it to Kenworth. Found out later, they never did.) I was given truck #306266, in a rush to pick up this now time sensitive load I grabbed my valuables and a few items needed to perform my job on this other truck I was out of there. Hopefully going to make it home for xmas with my 6 kids.(I didn't, I had 4 hrs with them)
